Kidney disease: Your elevated creatinine and low GFR are likely due to diabetes damaging your kidney. It is imperative that you control your diabetes closely to avoid further damage. Your doctor can also prescribe medication to protect your kidney from diabetes damage. If you have high blood pressure, that also needs to be controlled tightly. Please discuss all these issues with your doctor.

Kidneys: Creatinine and efgr reflect kidney function. This may be related to your diabetes. Asthma doesn't have anything to do with that, and thyroid probably doesn't either.

Diabetes : The most common cause of your renal insufficiency will be because your diabetes is not under control. Hemoglobin a1c less than 6, BP tightly control, may be the use of an ace inhibitor under doctor follow up. Check with your doctor.
