Usually a virus: If acute, the most common reason is a viral infection. Voice abuse or overuse can cause hoarseness, but not sore throat or cough. Laryngeal nodules can cause hoarseness but no cough. Allergies may cause hoarseness and cough, but no sore throat. Chronic hoarseness should be thoroughly investigated by ENT doctor particularly in a smoker.
