Lots of reasons: Assuming you mean competition between practicing dentists, you'll get many different thoughts about this. One thing is the distribution of dentists. Rural and inner city areas are largely under served while suburban and larger towns have ample supply of practicing dentists. Where supply exceeds demand you'll see more competition. In rural areas or inner city areas this is much less common.

Capitalism : Dentistry is not an exact science even though most dentists are licensed or go to accredited schools. There are many differences between the knowledge and expertise of dentists. That's why some feel that they can charge more and rightfully so. Do not look at dentistry as a commodity. Not all fillings crowns and dentists are the same. Research and shop around.

Marketplace competitiveness is a factor of license verses capitalism .. There is a floor and only some can rise above the education requirements & degrees necessary to compete. If you believe that your dental health must fit inside the dental insurance limits, a third party will make your choices.
