Mulitple things: Decay, trauma, infection, are the most common. Sometimes recession and exposure of the root surface of the tooth will cause some pain as well. It depends on the symptoms which one is likely the cause.

Toothaches: I agree with dr. Richards. There are two basic mechanisms that cause what is perceived as a “toothache”. Exposed dentin from decay, fractures, abrasions, and erosions can be transmitted through internal tubules inside the tooth to the nerve of the tooth or…. Pain can emanate from around the root of the tooth from an inflammation or infection of the periodontal ligament or bone.

A good question...: The cause can be most anything. It may include an infection, a leaking filling, deeper decay, trauma from clenching or grinding, or something else. I do recommend seeing a dentist who can sift through these possibilities to determine what is happening in your situation.

Most common cause is infection. But can also be caused by fracture, gum recession, parafunctional habits, traumatic occlusion, and so many other reasons. You need an oral cavity detective. Please see your Dentist.
