Surgical evaluation: Sounds like you need to be seen by a spinal surgeon. It is unclear if you actually require surgery, since that can only be indicated by a qualified surgeon. Make sure that you have copies of your prior treatments, so the doctor can get an appreciation of what treatment options should be considered.

Perhaps: I would agree with dr. Hyde and would emphasize the importance of obtaining all of your treatment records especially pain management including both the injections that you have had done and response to them. Although you presumably have multilevel radiographic pathology, the cause of your pain may only be from 1-2 levels and treatment should be directed at the source of your pain not the x-ray.

At this point you : Ned to be evaluated by a spine surgeon for your options . If you smoke, stop. If you are overweight, start dieting as both issues affect your treatment outcome with or without any surgery if indicated.
