Sounds atypical: Description sounds atypical but should have appropriate evaluation bya cardiologist. If negative, then would refer to a rheumotologist for costrochondritis.

No: If you seen dr for years, I am sure all cardiac work up was done multiple times (if not- find another doctor), i would worry more about orthopedic problem, like a shoulder issue, or maybe even GI problem.
