Unlikely: Appendicitis does occur that young but is rare. An exam by a physician is the best way to determine whether a further test is necessary. If you are worried that things are getting worse, a recheck is always the best option.

Not: Most likely not, any possibility of appendicitis would not leave doctor's office except to er or hospital, if the constipation did not resolve, then pain and vomiting will continue until you do something to resolve the constipation. But with the toddler if in doubt bring to er- safer that way.

Imaging: If he is still in pain and vomiting, monitor for fever and follow up. He may need an imaging test and other medical treatment for constipation.
