13 weeks pregnant at 40 yrs old. 2 miscarriages before at 10 weeks. what extra precautions should i take or of what problems do i need to be aware?

None : If you had ultrasound and saw a heartbeat - risk of loss is less than 2%. Also, you are almost in second trimester. Nothing to do . Some may use baby Aspirin and some may use Progesterone up to 14 weeks. Ask your doctor first before using any of these medications.
