Permission: In the us, a parent's authorization is required before a minor can be seen for any dental procedure. Once that permission is obtained, they can schedule their own appointments.

Legal majority: In most states have to be 18 to make health-care decisions (including appointments) for yourself. That said, if given permission by responsible adult a minor can schedule appointments as long as responsible adult agrees to the treatment plan and agrees to pay the bill.
