See below: Hey hiatal hernia is one part of stomach is above your diaphragm and in your chest. If your hernia is large, most of your stomach can be in your chest taking up space. This can cause chest pain. Sometimes the nerves in the chest can refer pain to the shoulder or arms.

Referred pain: Hiatal hernia has stomach up into chest, up through the hiatus (space) in the diaphragm that lets esophagus enter stomach-can cause pain. Irritation of diaphragm can cause pain . On top of shoulder. Esophagus and heart can have same innervation (upper thoracic) refers pain to inner aspect of arm (like heart attacks can).

Referred pain: The nerves that control your diaphragm muscle come from up in your neck. So when those nerves get irritated you feel the pain where they come from, ie your shoulder. Your whole is body is connected in ways that we don't often think about..
