Socks not shoes: Wearing compression socks makes the biggest difference with varicose veins. The best type to get are graduated compression, so they give more squeeze at the foot and ankle. Shoes should be comfortable, so that you stay active. Not the shoe itself, but the activity helps the veins.

Compression socks: The best answer is to see a vein specialist to treat the disease. Much easier process now, in office, no vein stripping. Flats: Besides graduated compression stockings as drs. Pak and morrison mentioned, you are better off wearing flatter shoes. High heels might accentuate the appearance of your calves but they don't increase the use of those muscles. Using calf muscles helps with venous return from your legs. Wearing high heels won't give you varicose veins but better venous return will make your legs feel better.

Stockings: Varicose veins are the result of venous reflux and stasis or stagnation of pooled blood in the veins of the lower extremities. Standing in one position for long periods of time can cause progression of enlargement of the veins. Compression stocking therapy is more important than specialty shoes. 20-30 mm hg knee length compression stockings are a good start for compression therapy.

Shoes and VV: Varicose veins are due to refluxing (malfunctioning) valves of the veins and the treatment is to close the leaking valves. Types of shoes that you wear will not improve the veins. There is also thought that high heels will disturb the weight distribution on the legs and lead to varicose veins. If you have a vein problem then see a vein specialist for recommendations.
