Hepatomegaly: There are several causes of liver enlargement (hepatomegaly). One common cause is fatty liver infiltration. This may be an early warning sign. The treatment is weight loss. Fatty liver can cause liver damage over time, sometimes leading to liver failure (worse case scenario) requiring liver transplantation.

Swelling a concern: The enlarged liver can be from several causes. The swollen ankles at the end of the day may warrant explanation now... I would be evaluated further by a good internist in the first instance.
