Payout limits: Read your policy. Most limit total payout for a lifetime, or an amount for a given illness, or the amount for certain drugs, etc. One more example of the fact that medicine is not controlled by doctors or hospitals but by insurance companies.

Limitations: All insurances have limitations. For example, most policy holders are not motivated to use their life insurance policy. In health insurance we are motivated to use the policy so there has to be a maximum provision to limit liability. A cap is a way for health insurance to limit liability and keep premiums affordable.
