Many reasons: The first thing to do is check with your surgeon.
Continued pain after a surgery can be from an improper diagnosis, not fully taking care of the problem during surgery, or recurrent tears at the shoulder.
Without knowing what you were going in for -- i.e. What was your diagnosis, it is difficult to tell if what was done to your shoulder was the correct thing.
Check with your surgeon.
