It can be many thing: It can be a splinter or broken piece of glass imbedded in the pulp of the toe. Perhaps it is bruised from overuse or it was injured. Some medical conditions such as diabetes can cause pain and abnormal sensations that start in the toes, such as diabetic neuropathy.
