When having a pelvic exam can the doctor accidently tear the hymen while looking at the cervix?

Yes: If the hymen is intact, with rare exceptions, the doctor should not be looking at the cervix. Virginal women cannot have cervical disease. If there is some reason to be looking at the cervix, such as unexplained bleeding or pain or exposure before birth to DES or trauma, the exam should be done by a doctor equipped with methods of exam that are less invasive.

Yes: If someone has tight or thin skin it is possible to tear the hymen during a pelvic exam. Usually these are small and will heal by themselves or with the aid of some estrogen cream.

Yes it is possible: Some hymens tear very easily.
