Many things: It could be a pulled muscle, pluritis (irritated lining of the lung), pneumonia , a problem with a disc in your back, or even a collapsed lung you really should have a good history with a doctor and a physical exam to evaluate this problem. I would not rely on an answer from an internet site to make your diagnosis.
