Unknown: Appendicitis is often treated primarily with antibiotics outside of us - where surgery is the back up plan. Given our legal climate, it is not likely to be widely accepted here anytime soon.

No one knows: That is an excellent question and no one knows the answer for a variety of historical reasons. After appendicitis was first described in 1886 there were three schools of thought about appendectomy - always, never and selectively. King edward vii developed appendicitis just before his coronation. The standard of care since then is to always remove appendix, so no data. See www.Appendicitis.Pro.
