Jaw pain: I am assuming you have consulted your primary physician regarding this problem which has been going for so long. I would advice consulting a orofacial pain dentist as you could be having temporomandibular disorders (chronic pain in the jaw). For starters, you could try massaging your jaw couple of times a day; it might help. Also, warm compresses for 10-15 minutes should help too.

Jaw vs ear: Could be ear or jaw. See ENT doc. if negative, see oral surgeon. Hard to discuss remedy without diangosis. B12 won't help.
