Who makes decision "not" to release your personal medical records?

You have rights: You can get copies of your records. practices don't always fully understand this, but it's a U.S. law. Hospitals and practices, can, however, charge you costs for the copies. Some places charge a lot, so find out the cost. You can ask for specific information, such as visit notes and lab test results only.
