Disruption of tissue: Ligament tears can be minor intrasubstance injury without disruption of continuity of fibers (grade i). There can be partial disruption of fibers with slight laxity (grade ii). There are others where there is complete rupture of the fibers with gross laxity (grade iii).

Ligamentous injury: Generally traumatic injury to the ligaments. Strong connective tissue which attach one bone to another bone. If there is a "tear" this is a grade 3 or complete disruption of this tissue.
