Yes: Tooth extractions must be carefully evaluated by an skilled dentist or orthodontist specialist as to the need for space and more importantly, what the effects may be towards facial support. Sometimes the facial profile is too full and would be more balanced with tooth extractions.

Swelling: There maybe swelling due to extractions but it is normally temporarily and they go away after a few days.

Possible: Having teeth extracted and ortho can change facial structure. Now with the techniques orthodontist use they can stimulate bone growth, extrude teeth all which will effect facial structure.
