There's a difference: A person can be alert, or even hyper-alert, and not be able to sustain mental focus. They may be scattered, highly distracted, preoccupied with an obsessive or delusional thought, etc. However, when a person is very sleepy and fatigued it is usually impossible to concentrate too.
