You must discuss this polypharmacy with your prescribing physician to sort this out. The beta blocker, metoprolol, can cause fatigue. You don't mention the strength and frequency of your dose. HCTZ, a diuretic, can cause potassium deficiency contributing to fatigue. And then there's weakness of heart muscle function, a notorious cause of fatigue&dyspnea . Only your own MD can alter doses of meds
