Need biopsy: A biopsy of the tongue mass is indicated in your situation. First an exam with your doc and ct scan imaging of your head and neck. The biopsy can often be done with needle. There is a real concern for malignancy in your situation. Don't delay.

Some lumps normal: The hard-to-see rear of the tongue has giant "circumvalate" papillae sheltering tastebuds. A few people have a central lump of tissue on the tongue where the thyroid gland originated. But it shouldn't hurt unless injured. If pain persists or it grows, certainly be checked by your dentist.
