Many things...: More history would be needed. For example, if the pain increases with deep breath, it could be an inflammation of the heart or lung lining. If it is tender to touch the area, may be muscle strain (there are muscles between your ribs), pain between cartilage and bone of ribs; sometimes shingles rash breaks out after the pain has been there for several days. Since very painful, check w doc.
