Exam: Physical examination with your doctor would be digital examination in the office and not an invasive procedure.

Rectal spasms: Rectal spasms are not usually caused by fistulas or hemorrhoids. Evaluation usually consists of an office exam with possible anoscopy (a 4 inch scope).
