Possible: A fracture involving the joint is more serious, however will often not require surgery. They will often monitor for proper healing and alignment.

Possibilities: May develop arthritis or a structural deformity. Really depends on type of fracture and how joint is involved. What has been done up until now?

Yes: If gravely displaced, i would surgical fix fracture. Any fracture that affects a joint will result in chronic pain and development of traumatic arthritis. The fracture will heal but every once in a while you may experience some pain and swelling in the toe. This arthritis will become more prevelant as you age, more so when you hit your fifties.
