Worse than mild: Usually not much- by only imaging it just means the spinal canal has become smaller and it may or may not create more or worse symptoms--in the cervical or thoracic regions in can cause cord changes or compression leading to myleopathy and myleomalacia or cord edema which requires surgery.

Depends: They, the person completing the dictation on your imaging study might be referring to "advanced" in comparison to your age. Alternatively, it might also mean that your stenosis (narrowing of the signal canal) is severe.
