If it is refractory and results in bleeding, perforation, stricture, or fails to respond to medical manangment then surgery is available. 50% of patients may require surgery at some time.

They are used for more severe disease. Some drugs target certain areas of the bowel when disease is localized. Some are taken by mouth, others delivered by enema, and others injected or given IV.
