Shoulder pain: Rest, ice and otc nsaids like Ibuprofen are a good start. If pain persists i would see your pcp. You may need an MRI and/or orthopedic evaluation.

Varies: If the pain just started, then i would use the rest, ice, and anti-inflammatory treatment. If the pain has been there for a while, it could be a sign of a labral (cartilage) tear or a rotator cuff tear. I would get an evaluation by an orthopedic specialist.

Diagnose: Doctor can examine for which soft tissue (ligament, tendon or muscle) is involved. This is hands-on evaluation because these tissues are mostly transparent to x- ray and mri. Treatment depends upon which tissue is injured. Physical therapy & cortisone are mainstay of non- operative care, but for best results need to know which tissue to treat. No way to diagnose from just MRI or x- ray.
