Sports medicine doc: You should see a sports medicine specialist who has experience treating throwing athletes, generally an orthopedic surgeon who has done a fellowship in sports medicine; throwing shoulders have a unique set of pathology, so if your problem needs pt/rehab you'll want a referral to an experienced therapist, if it requires surgery you'll want someone with that expertise.
