Testicular failure: Although it is difficult to interpret you question due to typos and abbreviations, it appears you have primary testicular failure. A high fsh and lh are indicative of this. What doesnt make sense is your normal testosterone. This should suppress lh. Are you taking exogenous testosterone supplements? This can also cause small testes. You need endocrinology and urology evaluations.

See a urologist: What this means is that even though your circulating hormones (fsh and lh) are high, your testis is not producing sperm. It is however making normal levels of testosterone. If both testes are small and you have a varicocele, you may benefit from ligation of the varicocele in terms of sperm production. A physical examination would be helpful. I strongly recommend you consult with a urologist.

Sperm ct.: A varicocele ligation is not going to help his problem. It appears he has primary testicular failure and at this time there is no treatment. He should also see an endocrinologist and rule out other forms of testicular failure.
