Spina bifida: Spina bifida occulta is a very common finding on x-ray. Usually not the cause of back pain. Most common cause of back pain in young athlete is stress fracture of pars intetarticularis. Back pain in a less than 17yo is considered a red flag and should be investigated. The goal is to get him pain free and then return to the game he enjoys.

Key word "occult": S.B. Occulta is a common condition-5% of general population, in whom it is almost always asymptomatic--hence "occult". There are many reasons for post-exertional back pain. Your son should be examined by a spine specialist.
