Severe ear infection: This could be a simple outer ear infection (otitis externa) with extension to the auricle, or an acute mastoiditis (severe middle ear infection with spread to the surrounding mastoid bone) which usually causes severe tenderness behind the ear and inside the ear. Whichever the diagnosis, the symptoms you described sound worrisome and really require an immediate intervention.
