Complications!: Tongue barbells can chip or fracture front teeth. Gums can be damaged by the rubbing of barbells requiring periodontal surgery. If the barbell becomes unscrewed it could be aspirated into the lungs. Infections can be quite severe & eventually lead to disfigurement. There can be significant blood loss & permanent neuralgia (pain). Safe? - the risk of complications is too significant.

See below: I am not a fan... Its a great way to leave your tongue wide open to infections. Think about it. Your placing a metal object through your tongue that the tissue cannot attach to... The result is a hole in your tongue. Since your mouth is full of bacteria someof these at some point in time will get into that hole.
