Acute vs chronic: Kidney stone pain (renal or ureteral colic) is typically sudden in onset and episodic and accompanied by other visceral pain responses like sweating and nausea, whereas pain actually attributable to kidney cancer (rare) would be more gradual and chronic in nature.

Stones: Small stone can cause terrible pain; usually kidney ca painful if larger tumor more advanced. Stones far more common.
