Scrotal pearl: Is likely a calcified appendix testis and of no cocern. It will come out when and if the hydrocele requires surgical correction. Hydrocelectomy will be indicated according to size or presence of discomfort. Suggest consult a urologist unless you have done so already. Good luck.

Nothing: These are likely calcified remnants of the testicular appendages and require no treatment. They will be removed if you have a hydrocele repair.
