It varies: There is no exact cpt code to describe this total procedure. A doctor can charge whatever they want to and send it to your insurance company for payment. The doctor will use an "unlisted" procedure code and send a description of the procedure and operative report with the claim. The insurance company will then determine what the benefits should be and what your portion will be. Ask first!
