Openly and honestly: Pain management physicians are well trained to help patients wean from pain medications. This is best done when one is open and honest with that physician (true with all interactions with physicians.) be sure to ask about what to expect and what to do if these occur. Good luck!

Off or change: I would seek out a board certified pain specialist or addictionologist. There should be no reservation in helping you come down and off of your pain meds. If done gradually it can be done successfully with little to no withdrawal. If weaning is difficult the the use of Suboxone could help. The doc would need a special dea number to use this med for detox. Check with them.

Why?: Your question is a difficult one to answer without knowing what the underlying pain condition is, and why you would like to taper off this medication. I wonder why you are not asking your current treating pain physician to help you with this. If you are indeed suffering from a chronic pain condition, you may need this treatment. Please discuss this with your treating physician or another one.
