Treat ALL contacts: You did not ask a question per se but i assume your wondering why the recurrence? Possibilities include: 1. Re-exposure to scabies 2. Incorrect treatment? Permethrin topical 5% should be applied to all contacts for 8-14 hrs and washed off. 3. This is not scabies but perhaps another type of arthropod bite, e.g. Flea or tick.
