See MD: Can't tell here. Some pigmented lips are normal (dark skinned people), some are not. Can be a physical exam marker for a genetic condition called peutz-jegher's syndrome; associated with a variety of polyps and some increased risk for malignancy (cancer). See your doc, and ask.

Which pigment?: Extra pigment outside the pink part of the lips can usually be lightened with topical bleaching medications or with intense pulse light (ipl) treatments. If you are speaking about the pink pigmentation of the lip, are you interested in making the pink lip smaller or do you have extra spots on the lips? You can reduce the size of the lips with surgery, but spots on the lips should be examined soon.
