Anxiety: If his md has ruled out cardiac problem, and gerd, the most likely is panic anxiety. Panic occurs in waves that may repeat throughout the day. A psychiatrist or mental health expert should be able to diagnose the problem and begin treatment which includes cbt (cognitive behavioral therapy) and medications. Prognosis is excellent for a motivated individual.

Somatic sx of depres: Some people have more body physical symptoms than mental symptoms of depression-anxiety. If chest pain comes in waves, frequently (added to other symptoms) the pain can mean anxiety attacks. Does he have difficulty to breather, heavy chested, oppression, long term fatigue? Low pep? Lack of motivation?. Depression can be treated and normalize his life, with combined medications.
