My right side under my ribs keeps hurting on and off. I feel a deep pulsating twitch inside my rib cage. What could this be? Why is it happening?

Diaphragm: As described, the problem is likely localized repetitive spasms of a portion of your diaphragm. This is the same phenomenon underlying hiccups, but affecting a smaller portion of this interesting muscle, near the ribs.
