Benign: Painful cramp can occur in any skeletal muscle in the body and by and large no cause can be identified. The condition is benign though very painful. If a specific muscle can be identified then Botox injection can help. A neurologist may be able to help you there.

Physical therapy: I agree with other answer. Along with trying botox, sometimes a good physical therapist can work with you for these sorts of problems.
