Many things: Long term joint swelling is associated with arthritis (rheumatoid and osteiod), repeated trauma (runner, especially on paved surfaces), anatomical joint anomalies and possibly postural asymmetry (one leg shorter than other i.E) long term swelling is an indication of a chronic, ongoing pathology of the area and should be evaluated by an orthopedist/surgeon.
