My baby is 5 months, and i've decided to start weaning and switch to formula. how many ounces should she get and how often?

It varies a lot: A very general guideline is about 3 oz. Per pound per day. This is very general. Some need more, some need less.

Stomach volume: Formula is a bit easier to measure than breast milk. I find the average kids stomach can fit about half their pound weight as ounces in a given feedinG.I.E.:8lb=4oz/feed, 10lb=5oz/feed. How many feeds they get in during a day is based more on their growth rate & energy requirements. Formula takes a little longer to digest so feeds may stretch out some.
