Autoimmunity: Ra is caused by an abnormal immune system causing a large and damaging autoimmune, inflammatory, reaction. Oa does not have this autoimmunity.

Friendly fire: In rheumatoid arthritis, the body's immune system thinks the lining of the joints (synovium) are an intruder (like a virus or bacteria). The immune system attacks (friendly fire) the joints causing pain, stiffness, fatigue, joint damage, and disability if not treated aggressively. Osteoarthrits is when the cartilage of the joint degenerates. The immune system is not directly involved in the proces.
