Can just stop: Normally, patients are instructed to just stop. The effects of Coumadin (warfarin) then "wash out" of the system over about 4 days or so restoring normal blood clotting in most patients. If quick reversal is necessary for a surgery or significant bleeding vitamin k (oral or iv) can be used. Special precautions are used in individuals at high risk for clotting who should not be blood thinners.

Doctor's instruction: I trust you are stopping warfarin at the instruction of your doctor. You should not be doing it on your own. If instructed by doctor to stop taking it, you should stop taking the medication. Your usual nutrition will provide sufficient vitamin k and you do not need to do any thing particular.
