Many possibilities: It's impossible to make a diagnosis with so little information. If you are 22, it's very unlikely that you've had a heart attack unless you use Cocaine or are diabetic. If you're feeling poorly, though, an er visit is reasonable. Otherwise, surely you should follow up with your regular doctor. Other possibilities might include peptic ulcer, gall bladder attack, pneumonia to name a few.

Go to ER: Symptoms such as these can be cased by many things, running the gamut between panic attacks to arrhythmia/cardiac pathology. At your age coronary causes are unlikely, but in addition to an ecg. An in-depth history and physical exam might be helpful. A normal ECG does not exclude an intermittent arrhythmia or cardiomyopathy. Orthostasis/vagal reflexes could be assessed . Go to er.
